2010年度診療報酬の改定内容が10年ぶりに引き上げられました。医科1.74％、歯科2.09％、調剤0.52％の引き上げで、従来の配分比率１：１：0.4という枠組みが崩され、今回は１：1.2：0.35になりました。配分比率の変更は、政権交代の影響が現れたと言えるでしょう。調剤については、原資が300億円ということもあり、改定項目は小幅にとどまりました。
しかし、薬局・薬剤師にとって注目すべき内容が盛り込まれました。後発品の変更時に、規格違い、剤形の違いも認められることになったことです。規格や剤形が違う後発品に変更する判断には、薬剤師の裁量にかかわる問題です。従来は、「変更不可」と記載されていない限り、変更を認めるという医師の了解もとでの変更でした。しかし、次回からは「錠剤」（口腔内崩壊錠）→錠剤（普通錠）、「10mg１錠」→「５mg２錠」というケースが出てきます。ある意味、処方権にもかかわる内容です。これまでも色、匂い、使用感の違いなどで患者さんからのクレームがありましたが、次回からは明らかに違う医薬品として認識される可能性が出てきます。患者さんに納得して頂ける説明ができるかどうか、規格違いでは患者負担にも差が出る可能性もあります。08年改定時にも「後発品使用促進のキーマンは薬剤師」と指摘されましたが、次回改定では、さらに「薬剤師の力量を問う」行政の意図が明確になりました。薬剤師にとっては、存在理由が問われる最後のチャンスでもあります。

Great article. When I was just starting out, I searched all over the web for about a year for methods on how to build muscles.
